Job Descripition:
Staff Nurse function in multiple roles, which include Dialyzer Reuse, Equipment Maintenance and Trouble shooting, Water Treatment Monitoring and Direct Patient Care. The Staff Nurse is the primary direct care giver for patients undergoing dialysis treatment. The job holder works closely with, and under the direct supervision of Unit in – Charge as an important member of the patient care team. Staff Nurse should understand the scientific principles of Dialysis; process of the Dialysis Treatment and to respond to the physical and emotional needs of patient undergoing dialysis treatment.
Medical Care:
- Assemble necessary supplies to conduct dialysis procedure; prepare dialyzer according to established procedures and the dialysis prescription
- Assemble and prepare the dialysis extracorporeal circuit according to protocol and dialysis prescription.
- Verify absence of residual sterliants.
- Test, monitor and machine functions, including alarms, conductivity and temperature. Set monitor and alarms according to unit and manufacturer protocols. Verify Dialyzer & documents.
- Discard dialysis supplies and sanitize equipment according to manufacturer and unit protocol.
- Ensure RO readings are in safe limits and water level is maintained for the subsequent shift & document the data.
Patient Care:
- Obtain and document pre-dialysis vital signs, weight and temperature.
- Inspect the patients Dialysis access, administer local anesthesia if required; insert needles, and initiate dialysis according to unit protocol and patient prescription.
- Document treatment parameters and communicate patient condition and issues to the unit-in-charge.
- Administer anticoagulant according to unit protocol and prescription.
- Measure and adjust blood flow rate according to established protocols and prescription.
- Calculate and adjust fluid removal rates according to established protocols and prescription.
- Discontinue dialysis and established hemostasis following unit protocol. Inspect, clean and dress access according to unit protocol. 8. Obtain and record post dialysis vital signs, temperature and weight.
- Monitor patients and equipment, response to alarms, and readjust treatment parameters as defined by established protocols and individual patient requirements.
- Change fluid removal rate and patient position and administer replacement saline as directed by the physician order or protocol.
- Respond appropriately to dialysis related emergencies such as hypotensive episodes, needle displacement or infiltration, clotting episodes, blood leaks, air emboli, etc.
- Initiate cardiopulmonary resuscitation (CPR) in the event of cardiac arrest.
- Collaborate with the unit- in-charge and Sr. Therapist in identifying and meeting patient education goals.
- Communicate emotional, medical, psychosocial and nutritional concerns to the unit in-charge.
- Maintain professional conduct, good communication skills and confidentiality in the care of patient
Process and Quality:
- Update patient vitals and dialysis data in Electronic Medical Reports and/ or specific files for current and future reference.
- Ensure adherence to NABH standards and other relevant quality standards in all processes. Staff and Facility Management 1. Assist Teammates in preparing patient schedules.
- Training new teammates for preparing, monitoring and maintaining dialysis machines and system.
- Monitor all the staff whether they are following the unit protocol if, not, report to the Unit in- charge.
- Ensure that reprocessing / dilutions etc. are done according to unit procedures and protocol.
- Ensure the facilities are provided for emergency resuscitation.
- To maintain the minimum stock in the emergency trolley.
- Check the emergency trolley on daily basis and ensure all the items are available.
- Ensure overall hygiene and waste management process, as per protocol within the dialysis unit.
- Assist in maintaining adequate staffing in the unit so as to ensure continuity of medical care at all times
Location: Sarguru, Mysore or Mangalore
Salary – 17k to 20k In-hand
Qualification- GNM or BSc graduate
